This funding opportunity announcement solicits cooperative agreement applications to investigate factors that influence well-being for young adults with rare disorders in the three areas of health, education or work, and social participation. Such research is needed for states’ cross-sectional surveillance efforts to document the outcomes and service use of these young adults.  CDC will consider spina bifida, muscular dystrophy, and fragile X syndrome as prototypes of rare disorders. Through retrospective, longitudinal observation, this research may discover reasons for barriers faced by some of these young adults and reveal ways to improve their well-being.
